In the AZV app, you will find your digital Health Insurance Card (digiZorgpas), which serves as proof of membership and must be presented to your healthcare provider to access services through AZV.

You can easily view information about your primary care physician, dentist, and pharmacy, and conveniently reach out to them via phone or email.

Parents have the option to include their children's membership card in the app. For children under 18 years of age residing at the same address, this can be done effortlessly from home. To add a dependent not living at the same address, kindly contact AZV at [email protected].
